Peculiarities of Baby Skin

Baby skin differs in some important anatomical and physiological characteristics from that of an adult. Before birth, babies’ skin is protected by a cheesy coating until the end of pregnancy. A baby normally doesn't need powders, oils, and creams to keep the skin smooth. Many babies have birthmarks that gradually fade away without treatment, although some may grow larger before they disappear and some birthmarks are permanent. Baby skin has the exact same number of layers as adult skin, but it is much thinner than an adult’s skin and more sensitive to exogenous influences. Baby skin is only a fifth of the thickness of an adult’s and therefore less resistant. The skin’s protective and barrier functions are not yet fully mature. In addition, the hydro lipid film and the protective acid mantle in babies and small children are still relatively weak due to the reduced activity of the sebaceous and sweat glands.

Horny layer and cells of the baby skin are not densely packed; therefore, the barrier function of the horny layer of the baby skin is still very limited compared to that of an adult. Thus, substances coming into contact with baby skin can migrate easily into the inner layers of the skin and absorbed easily. The reduced pigmentation of the baby skin leads to a higher UV sensitive as the melanocytes are present but less active.

The baby skin compared to an adult is especially sensitive to chemical, physical and microbial influences and prone to drying out. The capacity to regulate body temperature is much lower than an adult’s due to: (I) a relatively large body surface area, (ii) the reduced sweat gland function, and (ii) a comparatively slow adaptability of the skin’s circulation system.

Skin provides the outer covering for our bodies and protects us from heat, light, injury, and infection. The skin regulates our body temperature by releasing fluids to cool us and keeping heat in when it’s cold. The skin also stores water, fat, and vitamin D, all things we need to survive and live. The water loss of the skin is based on two mechanisms: active perspiration and Trans-Epidermal Water Loss (TEWL). TEWL plays the greatest role, especially in premature babies.

Special attention should be paid to the skin in the area normally covered by nappies. The baby skin can become inflamed due to (I) mechanical stress, (ii) combined with the irritating action of urine, feces, sweat, and (iii) the confining nature of the nappies. The action of bacteria on urea (ammonia formation), tryptic enzymes, due to raised pH, are activated which produce a skin-damaging contact noxa. Fecal bacteria and fungus Candida albicans play an important role in the etiology of nappy rash.
